An initial prescription is not needed for a patient to schedule a free evaluation. However, if moving forward with a device, prior authorization with the patient’s insurance may be required. At that time, a prescription and doctor or physical therapy notes might be needed for approval.
We accept all Indiana Medicaids and most private insurance carriers.
